Organ Scholar Program Hosting

I have read and understand the terms and conditions of the host participation in the AGO organ scholar program. I have secured the organ scholar housing I described in this application. I have secured a financial commitment from the AGO chapter listed in this application. I have secured the financial commitment and health insurance coverage from the host institution listed in this application. I certify that I am entitled to sign on behalf of the host institution and that the host institution will abide by the terms and conditions. I understand that failure to do so will result in termination of the scholarship.
