Before starting this form, please be sure you have at the ready the following items needed for upload during the application process:
- A digital file of the chapter’s complete financial statement (e.g., balance sheet and income/expense report) for the most recently completed year
- All names of proposed faculty
- A digital file (.PDF, .rtf, .doc, etc.) listing all proposed instruments and sites for teaching, practice, or recitals
- A digital file which is a scan of the “Statement of Approval” signed by the dean of the chapter. (Form is found on the lower half of the third page of Section 6.1.1 of the POE Handbook.)
Please note: The deadline for application to host a POE event is February 15, in the calendar year prior to the year in which an event will take place. For example, the chapter application to host a 2022 POE event must be received by February 15, 2021. The deadline for has been extended to April 15, 2021 for 2022 POE site proposals.
